Sarasota Healthcare Access provides low- or no-cost medical care for
eligible people. The program assists patients in establishing a
”medical home” for primary care, specialty care, and dental services.

Case Management
A case manager will determine if you are eligible for the program and
refer you to the appropriate provider for care as follows:
- Age 50-65 - Center for Healthy Aging, Senior
Friendship Centers (Sarasota
or Venice, depending on where
you live)
- Age 18-50 - Adult Health Center at Sarasota County
Health Department (Sarasota,
North
Port or
Venice, depending on where
you live)
- Age 0-18 - Child Health Center at Sarasota County
Health Department (Sarasota,

you live)
