Introduction

The Health Provocateur Project (HP) provides systems-level planning to complement CHIP's grass-roots, community-based activities.

 

Purpose:  To maximize the effectiveness of the local health system in addressing the broad health needs of the population in Sarasota County.

 

Process:  HP convenes major institutional stakeholders of the local health care system, including hospital CEOs, Health Department leadership, senior/mental health services providers, and Medical Society representatives. Provocative ideas challenging prevailing thinking are encouraged in a trusting and supportive environment.  National consultants and health care systems experts will also lend their voices to HP's efforts.

Design Team
The Design Team is the team of collaborators from 12 regional hospitals and health departments who have created a new model to improve services to the uninsured.  With funding from the State, Low-Income Pool Funds, the Health Department is hiring a case manager and pharmacy technician to help the uninsured have access to primary care services and free or discounted medications.  This Ad hoc Team has met 4 times since its creation in August 2006, and has become the Advisory Council of the Sarasota Healthcare Access program.

Sarasota Healthcare Access
This project is designed to help uninsured residents secure access to primary care services and provide options to Emergency Room care for the uninsured or Medicaid recipients needing non-urgent services.  The Sarasota County Health Department has hired a coordinator, case manager and pharmacy technician to coordinate among local hospitals and other safety net providers and Health Department services.

Sarasota Low-Income Pharmacy Team
Meets 3rd Tuesday of each month, 3:00 pm, location TBA
In response to findings from local hospitals showing that many emergency room visits are for medication refills or access to medications for the uninsured, the Health Provocateur group created the Low-Income Pharmacy Team to address this issue.  This team is comprised of representatives from all Sarasota hospitals and the Sarasota County Health Department, Bayside Center for Behavioral Health, Genesis Health Care, Senior Friendship Centers, St. Vincent de Paul Society, and Community Aids Network. This group is in the process of creating a Community Pharmacy to serve the uninsured and low-income residents of Sarasota County.
